Health Insurance for Nomads & Long-Term Travelers Leaving Japan
เนื้อหานี้ยังไม่มีในภาษาของคุณ
Once you deregister and leave Japanese employment, Japanese public health insurance ends and nothing replaces it automatically. Credit card cover is built for trips of a fixed maximum length that start from a home in Japan, so long-term travelers and nomads usually look at three other categories: Japanese long-stay travel insurance, offshore nomad travel medical policies, and international private medical insurance. This guide explains how those products work, what they leave out, how hospital bills get paid, and where Japanese insurance law draws a line.
The Post-Deregistration Reality: Zero Japanese Coverage Overseas
Section titled “The Post-Deregistration Reality: Zero Japanese Coverage Overseas”What Ends When You Deregister, and What Does Not Replace It
Section titled “What Ends When You Deregister, and What Does Not Replace It”National Health Insurance (NHI, Kokumin Kenko Hoken 国民健康保険) is tied to having an address (jusho 住所) in Japan. Membership is lost from the day after that address ends and is regained only from the day a new address is established.1 Cancelling NHI or employee health insurance and timing a new policy’s start date are covered in the guide to cancelling National Health Insurance and Shakai Hoken.
The NHI overseas reimbursement scheme (Kaigai Ryoyohi 海外療養費) does not follow you either. A city guide describes it as a scheme for people with an address in Japan who travel abroad for a short period, not for people who live abroad for a long period; those judged to have moved abroad file the moving-out procedure and are outside it.2 The details belong to the guide to senior healthcare and medical insurance abroad.
Japanese embassies cannot pay your hospital bill. The Ministry of Foreign Affairs (MOFA) states that embassies and consulates cannot negotiate with hospitals, bear medical or transport costs, guarantee payment, or advance money.3 What consular staff can do is covered in the guide to emergency support from Japanese embassies.
What Foreign Medical Care Can Cost
Section titled “What Foreign Medical Care Can Cost”MOFA’s own travel insurance guidance puts the burden squarely on the traveler. It says medical costs and the cost of transporting an injured person “must be prepared and borne by the traveler”, and calls cover for injury treatment, illness treatment and rescuer costs (kyuenshahiyo 救援者費用) essential.4 For a major accident it lists medical costs that may reach several million yen per person in serious cases, several million yen to dispatch a medical team for a transfer, and tens of millions of yen for a rescue team with a charter aircraft.4
One insurer’s own claims data, not industry statistics, shows the same pattern (as of October 2026). JI Accident & Fire Insurance (ジェイアイ傷害火災保険) reported that for April 2025 to March 2026 it paid claims at a rate of about 1 in 22 insured persons (4.5%), with treatment and rescue costs the largest category.5 Its two largest payouts were ¥25.3 million each, both in Hawaii.5
Who This Guide Is For (and Who Should Read Elsewhere)
Section titled “Who This Guide Is For (and Who Should Read Elsewhere)”This guide is for people who leave Japanese cover behind and do not join a destination public system straight away: long-term backpackers, sabbatical travelers, digital nomads, remote freelancers and working holiday travelers in their first months.
Some readers have dedicated guides: retirees (senior healthcare abroad), company transferees (overseas assignments) and students (study abroad planning). People who keep their Japanese address for a short absence keep NHI, as the guide to temporary absence versus emigration explains.2
The Credit Card Insurance Trap: Why 90-Day Coverage Fails Relocators
Section titled “The Credit Card Insurance Trap: Why 90-Day Coverage Fails Relocators”How Card-Attached Overseas Travel Insurance Is Structured
Section titled “How Card-Attached Overseas Travel Insurance Is Structured”Card-attached overseas travel insurance (クレジットカード付帯保険) covers a single trip of fixed maximum length. The issuer facts in this section are each issuer’s own published conditions, not a comparison. JCB covers accidents within 3 months of departure (2 months on some cards), counted from leaving your residence in Japan until returning (as of October 2026).6
Mitsubishi UFJ NICOS limits its cover to 90 days from the day after leaving Japan (as of October 2026).7
Cover is either automatic or usage-based. Automatic cover (jido futai 自動付帯) applies simply because you hold the card; usage-based cover (riyo futai 利用付帯) applies only if you meet issuer conditions, such as paying airport transport, flights or a package tour with the card (as of October 2026).6 Mitsubishi UFJ NICOS now requires that payment before you leave Japan, for departures from 1 December 2025.78
Several issuers have tightened card cover to usage-based conditions in recent years, including Sumitomo Mitsui Card for partner cards from 1 October 2022.9 The General Insurance Association of Japan (GIAJ) warns that card cover “may not be sufficient”.10
Why It Breaks Down for Open-Ended Travel
Section titled “Why It Breaks Down for Open-Ended Travel”On day 91 of an open-ended trip, card cover has typically run out. Both issuers above cap each trip and define it as starting from a home in Japan, and no issuer page reviewed offered a way to extend beyond that cap (as of October 2026).67
The limits are also modest. One Mitsubishi UFJ NICOS card lists ¥1 million each for injury treatment, illness treatment and rescuer costs (as of October 2026), against MOFA’s warning that serious treatment alone can cost several million yen.74
The issuer pages reviewed state no explicit “resident in Japan” condition beyond the trip definition, so check your card’s own terms. Whether you can keep the card account at all is covered in the guide to retaining Japanese bank and brokerage accounts.
Where Card Cover Still Helps
Section titled “Where Card Cover Still Helps”Card cover can work as a short bridge or a second layer. JCB explains that with several cards, treatment-cost claims are shared across insurers up to your actual costs rather than capped at one card’s limit (as of October 2026).6 GIAJ adds that separate travel insurance can top up only the missing benefits, though for a trip with no fixed end the per-trip caps above still apply.1067
Nomad Travel Medical Insurance vs. Comprehensive Expat Health Insurance
Section titled “Nomad Travel Medical Insurance vs. Comprehensive Expat Health Insurance”Japanese Long-Stay Overseas Travel Insurance (Kaigai Ryoko Hoken)
Section titled “Japanese Long-Stay Overseas Travel Insurance (Kaigai Ryoko Hoken)”Japanese overseas travel insurance (Kaigai Ryoko Hoken 海外旅行保険) covers the period from leaving your home for a trip abroad until returning.11 For study or postings the period can be set long, and some insurers arrange cashless treatment and medical interpretation.11
The key limit for this guide’s readers: it is not sold to people with no plan to return. GIAJ states that the insurance “cannot be contracted if there is no plan to return to Japan or if you will live abroad permanently”.11 Tokio Marine Nichido and JI Accident & Fire publish the same restriction, and JI also refuses policyholders not living in Japan, people already abroad, and trips for local employment or permanent settlement (as of October 2026).1213
Maximum terms vary by insurer and purpose (as of October 2026). JI allows cumulative periods of up to 2 years for long stays, 3 years for working holidays, and 5 years for study or postings, extended at most one year at a time, subject to review and only before the policy ends.14 Tokio Marine Nichido’s and Sompo Japan’s online products cover trips of up to 31 days, and their long-stay maxima were not found on public pages.1215
You must generally buy before you leave. JI bars insured persons who have already left home or are already abroad, though existing policies can be extended online from abroad before expiry (as of October 2026).1316
Offshore Nomad Travel Medical Policies
Section titled “Offshore Nomad Travel Medical Policies”Offshore nomad travel medical policies are sold online by foreign insurers or agents. Published terms share several traits: purchase while already abroad, a focus on new and unexpected conditions, limited home-country cover, and a fixed maximum of about 12 months per policy or plan (as of October 2026).1718192021
They are not designed as permanent health insurance; Genki itself warns that its travel product needs other long-term cover to return to (as of October 2026).21
International Private Medical Insurance (IPMI) / Expat Major Medical
Section titled “International Private Medical Insurance (IPMI) / Expat Major Medical”International private medical insurance (IPMI) is annual, renewable health insurance for people living outside their home country. Allianz Care describes a core inpatient plan plus optional benefits such as outpatient and dental, with an area of cover such as worldwide or worldwide excluding the USA (as of October 2026).2223
IPMI uses medical underwriting. Allianz Care’s explainer describes full medical underwriting, where you disclose your history and the insurer may apply exclusions or surcharges, and moratorium underwriting, where pre-existing conditions wait 24 months symptom- and treatment-free before they may become eligible.24 Some conditions may never be covered, and moratorium availability depends on location and local regulation.24
IPMI is the category built for living abroad rather than traveling. Allianz Care describes it as designed for people who live, work or study outside their home country, and Genki notes that its own IPMI product does not replace statutory insurance a destination may require by law (as of October 2026).2325
The Japan Legal Angle: Buying From an Insurer With No Japanese Branch
Section titled “The Japan Legal Angle: Buying From an Insurer With No Japanese Branch”Insurance Business Act Article 186 restricts insurers with no branch in Japan. A foreign insurer that has no branch or similar office in Japan must not conclude insurance contracts relating to persons who have an address or residence (jusho 住所 or kyosho 居所) in Japan, except contracts excluded by Cabinet Order.26 Under the Civil Code, an address is the base of your life, and a place of residence can be treated as your address where you have no address in Japan.27
The exception that matters is narrow. The Enforcement Order and Enforcement Regulation exclude contracts covering injury, illness, resulting death and baggage of an overseas traveler during the “overseas travel period”, which the Act defines as the time from leaving your residence for a trip abroad until returning to it.262829 An older, undated FSA page similarly lists overseas travel insurance among products a foreign insurer can conclude without a branch.30
The test applies when the contract is concluded, and renewal is a caution point. A February 2021 release by the Financial Services Agency (FSA, 金融庁) notes that if no insured person has an address or residence in Japan when the contract is concluded, a later move to Japan does not breach Article 186(1), but renewing while an insured person has an address or residence in Japan may require permission.31
Outside the exceptions, permission is needed before applying. A person who intends to apply to such an insurer for such a contract must obtain the Prime Minister’s permission by the time of application.26 The Act lists cases where permission must be refused, including where an equivalent or more favourable contract is easily available from a licensed insurer.26
Penalties fall on both sides. An insurer that concludes a prohibited contract faces up to 2 years’ imprisonment (kokinkei 拘禁刑) and/or a fine of up to ¥3 million (Article 316(iv)), and a person who applies without the required permission faces a non-criminal administrative fine (karyo 過料) of up to ¥500,000 (Article 337(i)).26
This guide does not assess whether any named insurer has a Japanese branch or licence; ask the insurer directly.
Typical behaviour by category is summarised below. Cells are generic patterns drawn from the published terms cited in this guide; your own policy wording always prevails.116713182122

Provider Examples: How SafetyWing, World Nomads, Genki, Cigna Global and Allianz Care Describe Their Products
Section titled “Provider Examples: How SafetyWing, World Nomads, Genki, Cigna Global and Allianz Care Describe Their Products”The profiles below repeat only each provider’s own published terms (as of October 2026). They are not a ranking or recommendation, prices are omitted, and none settles whether the insurer may contract with a person in Japan under Article 186.
Nomad Travel Medical Examples (SafetyWing Nomad Insurance, World Nomads, Genki Traveler)
Section titled “Nomad Travel Medical Examples (SafetyWing Nomad Insurance, World Nomads, Genki Traveler)”SafetyWing Nomad Insurance has two plans (provider terms, as of October 2026). Essential is travel medical insurance for “new, unexpected issues while traveling”, up to 364 days at a time, with a USD 250,000 overall limit and up to 30 days of home-country cover per 90 days; Complete is described as full health insurance, renewable for life, with no home restrictions.1718 SafetyWing names SafetyWing Insurance I.I., a Puerto Rico domiciled insurer, as underwriter of certain of its products, and says both plans can be bought while already abroad; Essential can be bought up to age 69 and Complete by applicants under 64.1718
For Essential, SafetyWing defines home country as where you principally reside and receive mail or, with no permanent address, where you would return after traveling (as of October 2026).18
World Nomads sells travel insurance, not primary health insurance (provider terms, as of October 2026). Its rest-of-world site offers policies of up to 12 months including extensions, purchase while overseas with a 72-hour waiting period, and cover for people under 66 at the start.321920 Its country-of-residence definition includes access to national or private long-term health insurance and a residential address; whether a deregistered traveler meets it is a question for the insurer.32
Genki Traveler is a monthly travel health subscription of up to 12 months (provider terms, as of October 2026). Genki states it can be bought while already traveling by people from any country up to age 69, with up to EUR 1,000,000 for unexpected medical care and a EUR 50 deductible per case (none for inpatient stays).21 Genki is the agent and group policyholder, and “Squarelife is the insurer”.21
IPMI Examples (Genki Native, Cigna Global, Allianz Care)
Section titled “IPMI Examples (Genki Native, Cigna Global, Allianz Care)”Genki Native is open-ended international health insurance, also insured by Squarelife (provider terms, as of October 2026). It has a one-month minimum and no maximum length, can be joined up to age 55, and is described as available “including nomads without any legal residence”.25 Pre-existing conditions are accepted, adjusted or declined after health questions.25
Cigna Global Health Options has Silver, Gold and Platinum tiers (provider terms, as of October 2026). Annual overall limits are USD 1,000,000 (Silver) and USD 2,000,000 (Gold), Platinum is described as unlimited, and outpatient, evacuation and dental are optional modules.22 Policies are underwritten by Cigna Global Insurance Company Limited in Guernsey.22
Allianz Care offers individual plans in three core tiers: Base, Enhanced and Signature (provider terms, as of October 2026). Evacuation and repatriation are standard in all three, and most policies accept applicants up to about age 75.23 The pages reviewed did not show the legal entity or regulator for individual plans.
Questions to Put to Any Insurer Before Buying
Section titled “Questions to Put to Any Insurer Before Buying”Ask every insurer the same questions, and get the answers in writing or from the policy wording.
- How do you define my home country, and what if I have no fixed address?183221
- What are the overall limit and deductible?1721
- Where will I be evacuated or repatriated, and who approves it?1822
- How are pre-existing conditions handled, and what is the look-back or moratorium period?182124
- Are outpatient visits, prescriptions, motorbikes and adventure sports covered?182122
- What is the maximum term, is renewal guaranteed, and can I buy or extend while abroad?18192025
- Do you bill hospitals directly?212223
- Do you have a branch in Japan, and how do you treat applicants who may still have an address or residence in Japan?2631
What Is Covered: Emergency Care, Evacuation, and Adventure Sports
Section titled “What Is Covered: Emergency Care, Evacuation, and Adventure Sports”Emergency Inpatient and Outpatient Treatment
Section titled “Emergency Inpatient and Outpatient Treatment”Japanese travel insurance pays actual treatment costs up to the sum insured. GIAJ explains that the illness must begin during the trip and be treated during it or within 72 hours after it ends, with longer windows for certain infectious diseases.11
Offshore products describe similar emergency cover with different claim mechanics (as of October 2026). SafetyWing Essential covers hospital stays, surgery and medication for new, unexpected issues, while Genki Traveler organises inpatient payments directly but has you pay outpatient visits first and claim later.1721
Medical Evacuation vs Repatriation: Where You Are Taken
Section titled “Medical Evacuation vs Repatriation: Where You Are Taken”Evacuation and repatriation are different benefits. Evacuation moves you to where adequate treatment is available; repatriation returns you to your home country. MOFA lists rescue teams, charter aircraft and medical escorts as separate cost items.4
Evacuation usually goes to the nearest adequate facility, not to Japan (provider terms, as of October 2026). SafetyWing describes “the nearest adequately equipped hospital” and Cigna Global “the nearest centre of medical excellence”, both with insurer approval.1822
Repatriation follows your policy’s home-country definition (as of October 2026). SafetyWing Essential pays a one-way economy ticket to your original location or home country with advance approval, while Genki Traveler ends once an insurer-paid repatriation is complete.1821 Senior evacuation memberships are covered in the senior healthcare guide.
Adventure Sports and Riding Motorbikes
Section titled “Adventure Sports and Riding Motorbikes”Motorbike and sports cover differs sharply between products. GIAJ lists unlicensed or drunk driving as a standard exclusion, and JI refuses insured persons who take part in dangerous sports.1113 SafetyWing requires a licence for the area and a helmet, with engines over 50cc needing an add-on on Essential, while Genki Traveler covers small scooters even without a local licence (provider terms, as of October 2026).1821
Injury That Leads to Disability: A Pension Pointer
Section titled “Injury That Leads to Disability: A Pension Pointer”Insurance pays for treatment, not long-term income. Whether a lasting disability could lead to a Japanese disability pension depends on your enrollment status, explained in the guide to voluntary National Pension enrollment abroad.
What Is Excluded: Pre-Existing Conditions, Routine Dental, and Chronic Care
Section titled “What Is Excluded: Pre-Existing Conditions, Routine Dental, and Chronic Care”Pre-Existing Conditions: Exclusion, Moratorium, and Full Underwriting
Section titled “Pre-Existing Conditions: Exclusion, Moratorium, and Full Underwriting”Travel products generally exclude conditions you already have. Japanese travel insurance excludes illness that began before the trip, although JI’s t@biho Prime plan covers the sudden worsening of a pre-existing condition (as of October 2026).1116
Offshore look-back periods differ (provider terms, as of October 2026). SafetyWing Essential looks back 180 days, Genki Traveler one year, and World Nomads excludes conditions existing when you buy or extend.183221 Under SafetyWing Essential, conditions arising under one 364-day plan count as pre-existing in the next.18
IPMI handles history through underwriting, and Genki Native decides case by case after health questions (as of October 2026).2524 Undisclosed history can lead to a refused claim.
Routine Care, Dental, Maternity, and Mental Health
Section titled “Routine Care, Dental, Maternity, and Mental Health”Dental disease and pregnancy are standard exclusions in Japanese travel insurance.11 JI offers a dental option on study and working holiday policies of 6 months or more, with a 90-day wait and 50% payment, and AIG’s cashless service does not cover dental treatment (as of October 2026).1633
Offshore products vary widely (provider terms, as of October 2026). SafetyWing Essential excludes check-ups, cancer and maternity, while Complete covers routine care, mental health support and cancer treatment, with a 10-month maternity wait.1718 Cigna Global includes mental health within core inpatient and daypatient cover.22
Chronic Medication and Carrying Japanese Prescriptions
Section titled “Chronic Medication and Carrying Japanese Prescriptions”Ongoing medication is usually outside travel medical cover, since SafetyWing Essential and Genki Traveler both exclude known conditions (as of October 2026).1821 IPMI and destination public systems are the categories that address regular treatment.
Carrying medicines across borders has its own rules. The Ministry of Health, Labour and Welfare (MHLW) advises that destinations may require a doctor’s certificate, limit quantities or need advance permission, and that medical narcotics need prior permission to take out of Japan.34 Carry an English explanatory document and keep medicines in their original packaging.34
Visits Back to Japan
Section titled “Visits Back to Japan”A deregistered person has no NHI during a visit to Japan. NHI resumes only from the day you establish a new address in Japan, and non-Japanese nationals must also be registered foreign residents.135
If your policy treats Japan as your home country, home-country limits apply to every trip back (provider terms, as of October 2026). SafetyWing Essential allows up to 30 days per 90 days for unforeseen conditions, Genki Traveler covers only emergencies in the first 6 weeks of each stay after at least 4 weeks abroad, and Genki Native offers a 182-day or full-year option in your country of citizenship.182125 For Japanese travel insurance, returning home ends the trip.11
Hospital Direct Billing and Guarantee of Payment (GOP) Protocols
Section titled “Hospital Direct Billing and Guarantee of Payment (GOP) Protocols”How a GOP Works Step by Step
Section titled “How a GOP Works Step by Step”A guarantee of payment (GOP, or letter of guarantee) is the insurer’s promise to a hospital to pay covered costs directly. Japanese insurers call this cashless treatment (kyasshuresu shinryo キャッシュレス診療): AIG’s assistance centre arranges treatment without payment at the hospital, while costs outside cover remain yours, and JI offers it only at partner hospitals (as of October 2026).3336
Offshore insurers describe the same idea (provider terms, as of October 2026). Cigna Global bills directly in-network after prior authorisation for inpatient and daypatient care, Allianz Care settles most inpatient bills directly, and Genki pays hospitals for inpatient cases handled by its emergency assistance.222321
The flowchart below is a generic pattern built from these insurers’ published descriptions; your insurer’s process may differ.

Pay-and-Claim: Documents to Keep
Section titled “Pay-and-Claim: Documents to Keep”When there is no GOP, your claim depends on paperwork. JI requires a medical certificate and receipts after paying at a non-partner hospital, and SafetyWing asks for receipts, itemised invoices, medical reports, prescriptions and test results (as of October 2026).3618 Deadlines are policy-specific: SafetyWing Essential accepts claims up to 60 days after the plan ends, and JCB asks card holders to contact its claims desk within 30 days of the accident.186
Deposits and Cash Demands at Private Hospitals
Section titled “Deposits and Cash Demands at Private Hospitals”Some hospitals ask for payment before or during treatment. JCB notes that a temporary out-of-pocket payment may be needed, and JI warns that without a payment guarantee a patient may not receive adequate treatment (as of October 2026).65 Embassies cannot pay, guarantee or advance these costs, while MOFA notes that travel insurance generally covers treatment, hospitalisation and emergency transport costs.337
Step-by-Step Administrative Procedure & Timeline
Section titled “Step-by-Step Administrative Procedure & Timeline”The timeline below is an illustrative planning sequence, not an official schedule. Visa-mandated insurance minimums are covered in the working holiday and digital nomad visa guides.
| Timing | Task | Applies to |
|---|---|---|
| T-3 months | List planned countries, activities (diving, motorbikes), existing conditions and likely visits to Japan | Everyone |
| T-3 months | Check destination visa insurance requirements | Visa holders |

| Ongoing | When you settle, review whether IPMI or a destination public system applies | Settling nomads |
Japanese Citizens vs. Foreign Residents: Critical Distinctions
Section titled “Japanese Citizens vs. Foreign Residents: Critical Distinctions”Both groups lose NHI once they no longer have an address in Japan.1 The differences lie in how insurers define your home country and what happens when you come back.
Provider definitions of home country mix residence, mail address, citizenship and return destination (as of October 2026).18322522 For a Japanese citizen these usually point to Japan; for a foreign national who has left Japan they may point to the passport country, an inference to confirm with the insurer.
| Point | Japanese citizens | Foreign residents |
|---|---|---|
| NHI after deregistration | Ends1 | Ends1 |
| “Home country” in offshore policies | Usually Japan | May be the passport country or the new base; confirm with the insurer |
| Repatriation destination | Usually Japan | Follows the policy’s home-country definition |
| NHI on returning to Japan | From the day a new address is established1 | Only as a registered foreign resident with a qualifying residence status35 |
| Japanese long-stay travel insurance | Only with a plan to return to Japan1113 | Only with a plan to return to Japan; not when leaving for good1113 |
| Keeps a Japanese address during a short absence | Remains on NHI; Kaigai Ryoyohi for short trips only2 | Same, as a registered foreign resident352 |
Registration rules for foreign nationals returning to Japan are covered in the guide to departure rules for Japanese citizens versus foreign residents.
Common Pitfalls & Regulatory Compliance Risks
Section titled “Common Pitfalls & Regulatory Compliance Risks”Provider limits and deadlines below are as of October 2026.
- Relying on card cover past its limit: typically about 90 days or 3 months per trip, shorter on some cards.67
- Not triggering usage-based card cover by failing to pay airport transport or flights with the card before departure.67
- Seeking a Japanese travel policy too late or with no return plan.1113
- Buying or renewing an offshore policy while you may still have an address or residence in Japan without checking Article 186: applying without a required permission can lead to a karyo of up to ¥500,000.2631
- Misreading “home country” and finding trips back to Japan barely covered.1821
- Undisclosed medical history leading to a refused claim.182124
- Riding without the required licence or helmet.18
- Assuming evacuation means a flight to Japan.1822
- No cash for a deposit at a hospital outside the insurer’s network.363
- Letting a policy lapse instead of extending before the end date.1420
- Trusting blog figures instead of the insurer’s current wording.
